Standard Grid Format

The standard Grid design is most normal arrangement having clear sections for OD, OS, SPH, CYL, and Axis.

They can be filled by a specialist in manually written or by electronic printing.

Understanding of Rx Notes:
SPH-circles will consistently have a (+) or (- ) sign.
CYl-Cylinder will likewise have a (+) or (- ) sign.
PD-pupillary separation can be written in various manners e.g 61 (signal PD) 30/31 (Dual PD).

Progressive 7 Bifocals Format

Standard Grid design with some extra sections is utilized for Bifocal and Multifocal glasses. Include or D.V/N.V segment will be incorporated and it’s worth will consistently has a (+) sign.

Understanding of RX Notes:

SPH, Cyl and Axis will have same signs.
DV – Distance vision or myopia. It will have (- ) sign
Include/NV-Near vision or perusing. It will have (+) sign.

Progressive / Bifocals Format

An uncommon type of prescription is prism Rx. It refers to the amount of prismatic power. It is usually needed to compensate for eye alignment. With standard grid format additional two columns are for prism and Base.
Interpretation of Rx Note:
Bo – Base out
Bi – Base in
Bu – Base up
BD/BDn – Base Down
